This form provides a mechanism to request custom services or ad-hoc assistance related to items that are not covered by the standard CSCS Service Catalogue, or are extensions to existing Services.   The content helps CSCS to gather pertinent information in a logical format in order for the request to be reviewed. Please note that the majority of custom Services noted below will incur additional costs over and above usual monthly CSCS charges.   Please refer to the New Service Enhancement request process for further information related to the steps involved. The following sections are to be completed by the Requestor who will be the designated contact for CSCS enquiries during the enhancement delivery:

Provide the justification for the proposed work, including details of expected business benefits.

How is delivery proposed and what resources are available to support the work?

List the key known risks that may prevent the project achieving the required outcomes - these could be budget, scope, timing, quality etc. Also any mitigations.
